FCCW Facilities Request Form Instructions

Improve the Facilities ability to respond to requests, suggestions, and observations.  Minimize confusion.
Download it here (FCCW Facilities Request Form Request) or grab one in the Church Office, on top of the mailboxes.
Any request for preparing rooms, special cleaning , observations of areas that need attention in our Church, or suggestions of how we may do better should be submitted on a FCCW Facilities Request Form.  Facilities will look over the requests, schedule and prioritize them.  In order to ensure that your request is completed when desired, please give us at least one week’s notice.  Urgent requests should be submitted directly to Ed Banzy.
Please fill out the required information at the top of the form. Include a sketch of any set up such as table types and positions and how many chairs.  Use the notes section for any special instructions, suggestions, or observations. Facilities will enter and prioritize the request in a calendar and action list.

We are hoping to develop an on-line request form to make it easier to submit.
